Please complete this waiver before participating in any activities at Precision Baseball LLC. Participants under 18 must have a parent or legal guardian complete the waiver. By submitting this waiver, you acknowledge and agree to the terms listed below.

Section 2 — Assumption of risk

I understand that participating in baseball or related activities at Precision Baseball LLC’s indoor training facility involves inherent risks, including but not limited to:

Being hit by baseballs, bats, or equipment
Slips, trips, or falls
Collisions with walls, netting, equipment, or other participants
Physical injuries such as strains, sprains, or impact injuries

By submitting this waiver, I voluntarily assume all risks associated with training, lessons, batting cages, pitching tunnels, camps, clinics, strength/speed programs, or any other activity offered by Precision Baseball LLC.

Section 3 — Release and waiver of liability

In consideration for being allowed to participate in activities at Precision Baseball LLC, I hereby release, waive, and hold harmless Precision Baseball LLC, its owners, employees, coaches, volunteers, and affiliates from any and all liability for injuries, damages, or losses—whether caused by negligence or otherwise—that may occur while participating in or being present at the facility.

Section 4 — Facility rules

I agree to follow all Precision Baseball LLC facility rules, including but not limited to:

No smoking or vaping anywhere inside the facility or on the premises.
Use equipment only as instructed.
No horseplay or unsafe conduct.
Respect all other athletes and staff.
No food, gum, or drinks on turf (water only).

I understand that violation of facility rules may result in removal without refund.

Section 5 — Photo and video release

I grant permission for Precision Baseball to use photos or videos of myself or my child for training or promotional purposes.

Section 6 — Cleanup and equipment

All participants must pick up all baseballs, equipment, and belongings after each session. Do not leave the facility, turf area, cages, or mounds in a mess for the next participant. Failure to clean up may result in loss of facility privileges.

Section 7 — Drop-in fee

$20 drop-in fee acknowledgment. I understand that all non-Precision players must pay a $20 drop-in fee per visit before my child participates in any activity at Precision Baseball LLC.

If the participant is under 18, I certify that I am the parent or legal guardian of the participant and agree to this waiver on their behalf. I accept responsibility for the participant’s actions and safety.
